Acrobat does not create all link all web links in a doc

After exporting a four chapter book in PageMaker 6.5 files to a PDF
file and opening Acrobat 4.0, everything, even linked contents and
Index works perfectly except for using Acrobat > Tools > Create Web
Links. On average, only about 50% of the the available URLs and Mailto
links are linked, and the message is given: "10 links were created on
Pages 1-60 (all pages)", which leaves another ten links not created and
not working. Sometimes there will be only one link created on a page
that contains several, sometimes two or more are created on a page and
other pages are missed completely.

Running the web link tool again, never succeeds in creating more links.
Running it and selecting a specific page with unlinked links brings a
message "No links were created." The links have all been
double-checeked and are valid. However, an interesting note: while
checking the linked links, some were found linked that would not work
because of obvious syntax errors and mispellings such as "xxx.come"
instead of "xxx.com" (so its not a matter of entering the URLs
incorrectly).

Am I just overlooking something, or doing something stupid?

Is there a way to create the links manually? in Adobe Acrobat 4.0?
