I think the OmniForm/Acrobat system would be the best. Once the form is made
or scanned, import or print it to Acrobat and you set up the form fields
withinin Acrobat IIRC. Regardless of what package you use to digitize or
create the form, if it is going to end up as a PDF, then form definitions
and such will ultimately have to be setup in Acrobat. I use Acrobat 5.05,
but never setup form fields so I can't give specifics, but I think it is
possible. The only thing that will come into play is security. Acrobat is
not only used for its portability, but because it is secure, you can keep
anyone from doing anything to it, including print. When you start letting
someone fill in form fields in an Acrobat form, the security system inherent
to PDF files will have to also be addressed and properly setup to allow
input.
This method will take more time (since you will have to setup each and every
field once in PDF format), but you are limited by trying to use a PDF as a
form filler, which is not its primary function. This is why OmniForm has a
seperate utility (for more $$$) that is a stand alone form filler, to
simplify such matters.
