That largely depends on what are you trying to do :)

If you want to just supply your own images/amimations and have screen
saver move them and do all the work, then, there are a couple of
freeware/shareware modules that will let you do so. This is an easy
way out.
If you want a screensaver that can do more than that, you'll have to
program it. Basically, a screensaver is your renamed executable with
few/no changes to it When windows 3.1 wants to start your screensaver,
it simply runs it with /s (I think) command line parameter. When
windows wants to call your screensaver's setup screen, it runs your
executable with /s parameter. It is up to you to take care of these
two.
Writing a screensaver in Visual Basic is particularly easy. However,
Visual Basic isn't really an optimum choice for graphics-intense
applications (although some commercial plug-ins can do wonders).
