kmail (KDE 2.1.1) attachment icons are ?

kmail (KDE 2.1.1) attachment icons are ?

I have KDE 2.1.1 and all other packages from RedHat rawhide
installed. Everything is working fine except when I receive
e-mail with attachment all icons are ? icons. Desktop and
konqueror do show all the correct icons for filetypes.

Is there a way to fix this or is it a bug?

1. Replacing the Red Hat icon with the KDE gear icon

How might someone replace the RedHat icon for the start application menu button
under the KDE 3.1 desktop with the KDE tri-gear icon?  The properties menu line
is missing for this panel button and for the desktop button also.  Can one
access the properties and change the icon?
