Help! Problem with SCO and Adaptec(SIDE SCSI-2/VLB/IDE/MIO)+Seagate(ST11200N)

                        HELP!

We have a big problem reinstalling SCO UNIX version 4.1 on box
with Intel 486DX2/66 after technical modernisation.

Steps we've passed through:
        - . . .
        - installing Basic Utilities;
        - installing Extended Utilities;
        - rebooting;
        - SCSI host adapter has recognized HDD as drive C:;
        - got the AMIBIOS Configuration Table;
        - got the three-line message:

Cannot open device: hd(40)/boot
Cannot open
Stage 1 boot failure: error loading hd(40)/boot

Now the questions:
   [1] What should we do?
   [2] What should we do?
   [3] What should we do?
   [4] What should we do?
   . . .

P.S. Hardware parts list also contains:
Seagate Hard Disk:
        Model:          ST11200N
        Serial Number:  TS909469
        Part Number:    947001-028
        Lot Number:     S-02-9429-6

One possible explanation.

RELEASE:  SCO UNIX System V/386 Release 3.2 Operating System Version 4.0
          SCO Open Desktop Release 2.0
          SCO MPX Release 2.0

PROBLEM:  I am unable to install SCO UNIX System V/386 on a SCSI hard disk
          over 1.0 Gigabyte in size attached to a 486 SCSI-CPU (Corollary EISA
          MP architecture) using the "ciha" device driver.  Though installation
          of SCO UNIX System V/386 appears to be successful, when I try to boot
          I get the following error message:

          Can not open device: hd(40)/boot
          Can not open
          Stage 1 boot failure: error loading hd(40)root

SOLUTION: This problem has been detected with RRD's (ROM Resident Diagnostic)
          prior to version 1.89.  If you are experiencing this problem, you
          may obtain the new RRD release by contacting your hardware vendor.
