How 2 mk a bootable system tape?

How does it fail?  Does it say the drive is busy or the tape is write protected?

We're having a similar problem on a J50- it will run a backup, but when we run a
mksysb (AIX 4.3) it thinks the drive is busy.

Just had it diagnosed over the phone by IBM as a drive failure.
