Check that it really is "configured" that is the state shown by lsdev is
"available" and not "defined". If it is defined then try to make it
available by using mkdev. If it is already available but mkvg cannot
access it, check that the drive is still connected to the machine. You
can do that by using the SSA service aids which you will find in the
diagnostic program (type diag - select service aids then ssa service
aids). You should be able to use the configuration verification service
aid to translate the hdisk number into a pdisk number then use link
verification to figure out which drive it is.
You might also try running diagnostics to all the ssa pdisks and
adapters to make sure you dont have a hardware problem.
