As mentioned in the subject line, I'm looking for documentation on
the three curses add-on packages under SVR4 called forms(3x),
menus(3x), and panels(3x).  Under AmigaUnix I've got a minimal set of
man pages that describe the prototypes for the functions but no
examples or overall conceptual guideline.

I'd prefer some form of on-line docs, but I'm not against going into
BookStop and looking for hardcopy (in fact, I'll do that later today).

The Prentice Hall book:

It is probably available in local bookstores.

However, a call to Prentice Hall uncovers an interesting point:  the
book is out-of-print; the SVR4 version will not be available until
September (any bets on _when_ in September? :-) and since the old one
is out-of-print it is not available from the 3 or 4 distributors I've
called.

To top it all off, the new version is Suggested List of $40.00 -- the
old one was $44.00!
