This works for me with a d-link 650.

My 650 works with
config 0x20 "ed0" ? 0x10
I'd bet this is ok with the 660, too, could you look for your config if
there are further problems?
BTW, an error I've made when setting the thing up was not setting
pccard_conf in /etc/rc.conf to the new config file, so it still used the
(stuffed) /etc/pccard.conf.sample, which broke pccardd. Checked this? ;-)
