Disk read errors? FAILURE?

This morning I was greeted by a bunch of disk read errors and kernel failure
messages on my screen.  They look like:

Apr 18 06:14:06 jingoro /kernel: sd0(ncr0:0:0): MEDIUM ERROR info:3cd0d1

    asc:11,0 Unrecovered read error
 ...
Apr 18 06:29:49 jingoro /kernel: sd0(ncr0:0:0): MEDIUM ERROR info:3cd775
    asc:11,0 Unrecovered read error
Apr 18 06:30:44 jingoro /kernel: sd0(ncr0:0:0): MEDIUM ERROR info:3cdac7

 ...

With several more of each deleted for brevity.  The disk should be in perfect
operating order...?  Is it?  Or could something else be causing these errors?

I'm running FreeBSD 2.1 stable vintaged roughly a month ago.  In the past
week I _did_ replace my memory.  Could it possibly be the culprit?

In the meantime I haven't rebooted my machine so I can see if this happens
more often...

It's your disk.

If it's new, better have it replaced.  Well, `synch mark error' --
perhaps it's running too hot?

You can try running ``scsi -f /dev/rsd0.ctl -e -m 1'', and turn on
automagic sector reallocation (top couple of lines in the editor).

Unless someone threw a blanket over my case that drive is the coolest drive
I've ever used.  It's well ventilated, even cool to the touch.

Sector reallocation was and _is_ on!  Someone suggested that it might be
my memory that I recently replaced.  I'm going to change that back and
see if it helps.

Then i would take this error message as a serious warning sign that
your drive needs to be replaced.

1. Solaris 9 - install - short disk read - boot failure (FYI)

This is on Sparc Station 5 - I actually called SUN on it, and here's
what they told me:

My OBP is too old, so I cannot boot of a partition over 1G (I guess in
Sol9, because in 8 I am doing it).
Anyhow - upgrade of OBP is a hardware replacement - so unless the box
is on contract (and not many ss5s r any more;) I am out of luck.

So.. the solution is to either split into multiple partitions, or stay
with Sol8 on old boxes. I did not ask about other, but I'd expect
pretty much anything older than SS5 to have this problem....

I will try reinstalling on different size partition, see what happens,
but if anyone wants sol9 to run on their old stations, be aware of
this I guess.
