What is Kernel Panic: EXT2-fs panic (Device 3/65)

Help.  I tried to install Slackware 2.1 and just as setup tries to
create a boot disk for me, it gives me the following error

Kernel Panic: EXT2-fs panic (device 3/65): ext2_read_inode:
unable to read inode block - inode=12, block=6

Can someone explain this to me?

Where can I find a trouble shooting guide that explains these error
messages?

 
 
 

1. Kernel panic: EXT2-fs panic (device 3/65): ext2_find_entry: buffer head pointer is NULL

Some time ago I found strange message:

Kernel panic: EXT2-fs panic (device 3/65): ext2_find_entry: buffer  head pointer is NULL

... and system hangs:(

I can simply make this situation (anytime I wish) by calling "man"
with asking ordinary help for some programs :(

What does this means? And what I can do for prevent this situation?
Is it v1.3.3 of Linux kernel specific or there's something other wrong?

P.S. All mine disk files readed ok (I maked full backup of mine HDDs without
any errors!)
