I have a Targa three button mouse which uses two button (argghh) microsoft
standard and has the possiblity of working in three button mousesystem mode.
On the DOS side this is accomplished by the mouse driver (there isn't any
hardware switch on the mouse).

Is there some kind of tool to put the mouse in three button mode (simply using
that standard in XF86Config results in the mouse not working at all) ?

I was having this problem as well.  I found that it was necessary to power up
the computer while holding down one of the mouse buttons to put it into 3-button
(mousesystems) mode.  It seems to make everything work OK (my mouse does not
have a 2-button/3-button switch on the side like some others do).  My XF86Config
uses Protocol "Mousesystems", and I think I've got a ClearDTR line in there
as well.

then try clearRTS also in your Xconfig file.
I need both on my sicos-mice
