i have a 3-button microsoft mouse in /dev/cua0

why can't i use the third button, none of the other features like Emulate
etc work

First, try /dev/ttyS0 instead of the cuaX devices.

Do the "other features" include a "Buttons 3" line in /etc/XF86Config?
Does the "Device /dev/<whatever>" setting point to the device you're
using? Is the "Protocol <whatever>" setting correct (note: just because
it's a Microsoft mouse doesn't mean you should use "Protocol Microsoft",
this protocol is for two-button mice only, IIRC)?
