Does your Xconfig specify 'mousesystems' type mouse?

mousesystems "/dev/mouse"

You could run `od -c /dev/ttyS0` to find out what characters your mouse
is spitting out for each button and for each kind of movement.  Make
sure that the middle button is actually generating *some* character(s),
and that they are different so that the middle button can be
distinguished from the other two.  Use /dev/ttyS1 for the second serial
port.  Maybe your mouse is broken.

I just tried this.  Earlier I tried "cat < /dev/mouse &".  I also
tried 'echo -e "\334"' to try to activate the mouse.  In all cases it
appears that the mouse's 3rd button is not working.  Either there is
some secret code to activate the mouse or like you suggest it's
broken.  Does anyone have a 3-button KeyMouse Model 3 mouse?  Does it
work?
